Registered Nurses provide skilled nursing care in the home under a physician’s directive. The nurse communicates with the physician to update or change the patient’s plan of care, discusses prevention strategies and coordinates patient care and services with the physician and other health team members.


  • Provide skilled nursing services by registered nurses in accordance with the physician’s plan of treatment, including but are not limited to:
  • Assessment and regular reassessment of the nursing needs of the client throughout the course of care and the development, implementation, review and revision of the nursing care plan
  • Providing nursing services including treatments, diagnostic and preventative procedures
  • Initiation of preventative and rehabilitative nursing procedures as appropriate for the client’s care and safety
  • Observation of physical and emotional signs and symptoms and reporting to the physician of all reactions to treatments, including medication administration
